There are various reasons that can explain why measures appear blank even if they are marked as completed. Please read through this guide to check to see if the root of your problem lies with any of these explanations. Generally it means that not enough questions were answered in order for the measure to score.
Basic Troubleshooting Steps
Navigate to the patient's page and click "Details" on the relevant session.
On this page, scroll down and select the gear icon for the relevant measure and press "Edit Responses".
Within this page, you can see the specific responses to questions. You can view this without submitting changes. If the questions are not answered, the patient submitted or closed out of the measure without filling them out.
